Diagnostic Value of Adenosine Deaminase Isozyme in Various Pleural Effusion




Abstract
Recent investigation have suggested that the measurement of ADA activities in various pleural fluid is useful marker for the diagnosis of the tuberculous pleural, effusion. But several diseases such as empyema, parapneumonic effusion, rheumatoid effusion also show high ADA activities. Therefore, high level of ADA activities is not differential point among various pleural effusions.
ADA exists in two molecular forms, a small molecular ADA isozyme and a large molecular ADA isozyme.
In this study we examine the molecular forms of ADA in various pleural effusion(tuberculous pleural effusion: 20 cases, empyema 7 cases, malignant effusion: 10 cases, transudate group: 5 cases) by gel filtration and polyacrylamide gel electrophoresis(PAGEI
The results were as follows:
1. The mean ADA actvity of tuberculous pleural effusion, empyema, malignant pleural effusion,and transudate groups is 105.1 U/L, 169.5 U/L, 20.0 U/L, 15.9 U/L, respectively.
2. The estimated molecular weight of large molecular form of ADA isozyme is 240,000 and of small molecular form of ADA isozyme is 38,000.
& On PAGE with active staining, tuberculous pleural effusion shows only large molecular form of ADA isozyme.
4. The empyema shows both large and small molecular form of ADA isozyme, except one case which shows only large form.
In conclusion, the measurement of ADA isozyme in pleural effusion may be of value in distinguishing between tuberculous pleural effusion and other causes of high pleural fluid ADA activity.
